1)  Complete "Shipping Details" section.  Use your street address only.  UPS does not deliver to P.O. Box addresses.   If you are unable to complete the order form on-line, print the blank form, fill it out, and fax it to us for processing.  Please type or print legibly.

 

2)  Select quantity and total for each item desired.   Add "qty/totals" for each item and enter in "Total" box.

 

3)  Select correct "Shipping & Handling" charge from drop down menu.   This amount covers UPS Ground shipment to your street address in the 48 contiguous states.

 

4)  Select "Special Handling" charges if needed.  These charges are in addition to the "Shipping & Handling" charges above.

UPS Next Day Air:

add $20.00
UPS 2nd-Day Air: add $10.00
Alaska/Hawaii/Puerto Rico: add $11.00

 

5)  Calculate Sales Tax (California addresses only)

 

6)  Add {a},{b},{c}, & {d} together and enter in "Grand Total" box.

 

7)  Press the "Submit Order" button once and wait for the confirmation page.  Submitting your order on-line will speed up the processing of your order.

 

8)  Print 2 copies of Order Form.  Save 1 copy for your records and send the other copy with your payment.

 

9)  Mail a check or money order payable to "ALC Imports" along with one copy of your order form to:

10)  If your order arrives at our office with incorrect payment, we will send you an e-mail message requesting additional funds.
